Directory Information Form

Mark your calendars!  Over the next 3 Sundays, March 10, 17 & 24, we will have a photographer there ready to take your picture for the new church directory.  The photographer will take pictures from 9:30 – 9:55 a.m and then again from immediately after the service until noon.  So please be prepared to have your’s taken one of those dates!  You MUST bring the completed Connect Card with the appropriate signatures.

Here is the “Connect Card” if you didn’t pick up a form at church.  Please complete it.

  • This form & photo are for Heritage Alliance Church to maintain clear, accurate and up-to-date records in order to serve our Church. The information we collect within these pages will be entered into our office database program. This will help us to provide updated, pictorial directories (digital & paper) for our congregation. It will assist us in providing information to our ministry leaders and pastoral staff. It will also assist our Heritage Alliance family in staying connected with each other, for the purposes of nurturing and serving our Heritage Church family and our surrounding community. Please note, the starred areas are only for the church office database only. Also please understand that you are under no obligation to include any information that you are not comfortable sharing. If you have included information in this form in an area not marked with a star and you would like it for office use only, please mark it with an x and this info will not be used in the directory. Please complete the entire document. You will need to print it out and sign in the necessary areas.

    Please print out this form & mark with an ‘X’ any other information you choose not to be included in the directory. Don't forget to sign the Directory Agreement and the Photo/Media Waiver. I agree that the above information and picture on file may be published in a Directory for Church use. (Signature needed)
  • Photo & Media Waiver

    On occasion, Heritage Alliance Church may take photographs or audio or video recording of you and/or your children as you are involved in activities. These may be used by Heritage to remember the events. These also may be used in church publications to let others know about our church, such as the Heritage website &/or Facebook pages. The local news may also be invited to photograph events to be used, distributed or displayed as the agents of the church see fit. I hereby expressly grant to Heritage Alliance Church the right, privilege & license to use a picture or likeness of myself &/or my child(ren) in any photograph, video production, or any other forms of media publication & to use the verbal or written statements or declarations of myself &/or my child(ren) for the purpose of publicizing, fostering & promoting the church & its programs.
