Heritage would love to put out a new church directory soon. However, to do this we need to make sure that we have updated information on all of our church family and most important, your permission for us to do so.
Could you please help us out and pick up a “Connect Card” from the info booth or children’s sign in table? Or just click on the link below to submit a digital form.
If you are sure that your information has not changed, just fill in your name at the top and over the printed info write – “no changes”. If there is some info that you do not want in the directory just note that beside the item. And please, sign the bottom as a release to allow us to publish your information.
There is also a couple of questions about involvement on the back of the card, we would appreciate if you would like to check that out as well.
And then just place the Connect Card in the offering basket or leave it at the information booth (or click on the link below).
